Confidential Insurance Intake
We care. You’re covered.
Answer what you can. We take it to our carriers and come back with real numbers.
Coverage terms and document names stay in English on purpose — that is how you will see them on your paperwork.
If any part is unclear, leave it blank and ask your agent. A blank answer is always safer than a guess.
This is a request for a quote. It does not bind, extend, or change any insurance. Nothing is covered until a carrier issues a policy and we confirm it in writing.
Do not cancel your current policy until your new one is confirmed in writing.
Choose what your client sees. Their copy opens locked to these settings.
Untick anything you would rather gather yourself. This is the main lever for keeping the form short.
The link carries only the settings above — never a name, VIN, address, or answer. Links get forwarded, screenshotted, and cached, so no client detail belongs in one.
When a client sends back the file from “Download my answers”, open it here and the whole form fills in as they left it.
Finished with one client and starting the next? Clear the device first so nothing carries over between people.
Your agent set these for you.
Contact your agent for a corrected link rather than working around it.
Pick one or more. The rest follows from this.
The rest of the form stays hidden until you choose a policy above.
Often right. But a vehicle titled to the business, or used for deliveries, usually belongs on the commercial policy only.
Enough to call you back. Use your legal name, the way it reads on your licence.
A licensed advisor will use these to answer your request. We save your name and number when you fill them in, so we can reach you even if you stop partway. Message and data rates may apply. Reply STOP to any text to stop texts. How we handle your information
Use the exact legal entity name. The wrong one can void a claim.
An uninsured subcontractor is charged back to you at audit as an employee.
Continuous coverage is one of the largest credits available. Your renewal date tells us when to call.
No commercial carrier quotes without 5 years of loss runs. Your current agent must provide them — ask today, they take about a week.
We can quote from here. The rest is detail a carrier needs before it can issue — you can type it, or an advisor can take it with you.
You are not skipping questions, only choosing who types them. On the next screen you can send your answers by WhatsApp, email or PDF, and you can come back to this step at any time and switch.
Your current declarations page, the drivers or property involved, and any claim in the last five years. Anything you do not know can stay blank — a blank is better than a guess.
Everyone of driving age in the household. An unlisted driver is a denied claim.
Someone can be excluded in writing — but then there is no coverage at all when they drive. Ask us before you sign.
Year, make and model is enough to start. Add the VIN if you have it handy.
One block per property. In Texas, the roof matters most.
Wind and hail usually carries a separate deductible, often a percentage rather than a flat sum. On the coast it may come from TWIA.
Tap through it. If you have a DOT number, that alone answers most of this.
One block per power unit. The VIN is helpful, not required.
This decides your cargo cover and which carriers will look at you.
Placarded hazmat is held to far higher federal minimums — often $5,000,000. Tell us what you carry and we will confirm the exact requirement.
Underwritten on what you do and who you do it for.
E&O is almost always claims-made: the policy that pays is the one in force when the claim is made, not when the work was done. Moving carriers without carrying your retroactive date forward erases every year you have already worked.
The last 5 years. Carriers pull a loss report regardless.
All of this can change before anything is issued. Unsure is a fine answer.
Premium is set by each carrier against your own record. Pick the protection you want; we come back with real numbers.
Built from what you picked. For each one, tell us where you stand. “I can get it” is a perfectly good answer — we just need to know what to chase.
Check the summary, then send it however suits you.
Easiest: save the PDF, then send it the way you normally message your agent.
If we asked for the file: use the second button instead. It saves a small
.json file we can open directly, which saves us retyping anything.
What has already reached us: your name, phone, email, ZIP, the coverage you asked about and your renewal date, so an advisor can call you back. What has not: everything else on these screens. Dates of birth, licence numbers, VINs, claim history and anything you typed in a notes box stay on this device and go nowhere until you send them yourself, with one of the buttons below.
You chose to let us collect the rest. Send what you have with any button below and a licensed advisor will call or message you, usually the same business day, to finish the details and start the quote. If you would rather type them after all, go back to “That is everything we need to start” and switch.
WhatsApp and email open with your answers already written in. Check it, then press send. If nothing opens, our WhatsApp is 832-775-2509 and our email is Insurance@kivulink.com.
A quote request, not an application. It binds nothing — keep your current policy in force until we confirm a new one in writing.
This intake form is a working tool. It is not an insurance application, not a binder, and not a
quote.
Answers stay in this browser until you export them.
